I'm the procurement manager for a 180-bed healthcare network. Over the past six years, I've documented roughly $2.3 million in annual purchasing across things that look nothing alike: infusion pump sets, CPAP machines, patient comfort supplies, and more than a few cameras. Total cost after the first year came out almost $800 higher than the $450 option. The lower quote was not lower. It just looked lower until the hidden costs showed up.

I built a simple cost calculator after getting burned on hidden fees twice. Is it perfect? No. But it forces me to write down every non-optional cost before I talk to a vendor. The question isn't whether we can afford X. The better question is what X costs us to make useful.

There are three different scenarios, and they need three different answers

I don't use one purchasing philosophy for everything. I use three, depending on what we're buying.

First, products you use once, or nearly once, should be bought with price-per-use in mind. Second, devices that run constantly or support clinical care should be bought with uptime and consumables in mind. Third, tools that create long-term output should be bought with workflow costs in mind.

Scenario 1: The cost-per-use decision

The Kodak or Fujifilm disposable camera question is a small version of the same math. If you're buying one camera for a trip, the difference between a Kodak and a Fujifilm disposable camera might be a dollar or two. That's a one-time price, not a real decision. But if your organization buys 400 of them for patient education kits, a small price difference becomes a line item. Their prices vary, but the real cost is how many changes per day each product requires, how much laundry it creates, and whether it causes skin damage that turns into an expensive wound-care visit. A product that costs 20 percent more but lasts one extra hour can reduce the number of changes and the total cost. In my opinion, this is a scenario where price-per-use beats unit price every time.

Scenario 2: The reliability-plus-consumables decision

Infusion pumps and CPAP machines are not the same product. But from a cost-controller perspective, they belong in the same group because their total cost lives in what happens after the quote.

When I buy an infusion pump, I'm not just buying the pump. I'm buying the supply chain around it: the administration sets that fit the pump, the batteries that may or may not be included, the service plan, and the software update policy. Two pumps with nearly identical numbers on the brochure can have very different costs over three years. One might use inexpensive IV sets that are already on contract. Another might require a proprietary set that only one distributor stocks. The pump with the lower sticker price can end up costing far more by the third year. This isn't about choosing a bad pump. It's about not confusing the device cost with the system cost.

CPAP machines are even trickier because patients use them every night. A machine's home isn't the box. It's the nightstand, the travel bag, the humidifier chamber, the mask fit, and the compliance software. I still kick myself for choosing a CPAP machine based on a lower quote without checking the long-term mask and filter cost. The upfront savings were maybe $6,000 across a batch. The replacement mask cost was 40 percent higher, and the same-day replacement option wasn't available for that unit. The risk of a patient going a week without a machine was not worth the savings. Scenario 3: The workflow-and-longevity decision

Second, what else must be true for this product to work? Does it need compatible supplies, a repair plan, software updates, or training? Every yes adds hidden cost.

Third, what happens if it fails? A failed disposable camera is annoying. A failed infusion pump can delay therapy. A failed CPAP machine can disrupt a patient's night and send them back to the clinic. The cost of failure is often the biggest number that nobody puts on the spreadsheet.

Sometimes the answer is clear: buy the cheaper item because the failure cost is small. Sometimes it is the opposite. In Q2 2024, I compared quotes for a one-year CPAP equipment contract. The lowest quote looked great on paper. But when I calculated the cost of delayed replacement parts and extra training, the second vendor was only about $2,800 more, and their response guarantee was written into the contract. The low quote was not actually low. It just looked low before the scenario was clear.

Personally, I don't believe any product is always the right choice. Not a Fujifilm camera, not a Kodak or Fujifilm disposable camera, not a particular model of infusion pump. The right choice is the one that matches the scenario. I wish I could give you a single answer. After six years and a lot of invoices, I'm not sure that answer exists.
